Case Manager

When patients and their families enter the healthcare system — as a result of a diagnosis, accident, or other life-changing event — case managers help them to make the most informed decisions. Case managers individually meet with families to understand their needs, dynamic, and financial situation. This helps case managers to personally explain the process of accessing and coordinating services such as physical therapy, occupational therapy, and nursing. Evaluating the resources available in the area and the patient’s insurance policy, case managers may also assist patients and families in accessing necessary medical equipment. 

Job Responsibilities

  • Assess the needs of patients and their families 
  • Offer emotional support to patients and families 
  • Organize appropriate support for patients, which includes coordinating other services and monitoring the success of those services once they are in place 
  • Create patient care plans 
  • Modify patient healthcare plans as needed
  • Coordinate with physicians and other medical providers

Qualifications and Skills 

  • High school diploma, GED, or equivalent experience 
  • Associate degree in a related field 
  • Bachelor’s degree in nursing, human services, or similar field preferred 
  • 2+ years of experience in a similar role or in the healthcare field   
  • Proficiency with word processing software
