UAS Assessor - Troy, NY 
 
HealthCare Support is actively recruiting for a full-time UAS Assessor to support members around Troy, NY. Schedule:
<ul>
	<li>Monday – Friday, 8:30 AM – 5:00 PM
	<li>Work-life balance: No weekends, no holidays, no night shifts
</ul>
 
Compensation Notes:

<ul>
	<li>$35.49 - $63.79 per hour
	<li>Annual Bonus Incentive 
	<li>Mileage & toll reimbursement
</ul>
 
Daily Responsibilities for UAS Assessor:

<ul style="margin-bottom: 0.0in;margin-top: 0.0in;unicode-bidi: embed;">
	<li>Assess and coordinate medical and support services across the continuum of care for complex, high-acuity populations to promote quality, cost-effective outcomes (approximately 2 or 3 per day)
	<li>Develop and manage personalized long-term care plans while educating members and families on available services and benefits.
</ul>
 
Required Qualifications for UAS Assessor:

<ul style="margin-bottom: 0.0in;margin-top: 0.0in;unicode-bidi: embed;">
	<li style="margin-top: 0;margin-bottom: 0;vertical-align: middle;">New York RN license
	<li style="margin-top: 0;margin-bottom: 0;vertical-align: middle;">4+ years of RN experience  
	<li style="margin-top: 0;margin-bottom: 0;vertical-align: middle;">Home Health or UAS experience
</ul>
