Community Resource Coordinator - Field 
 
HealthCare Support is actively seeking a Community Resource Coordinator - Field to fill an opening with a Fortune top 25 Managed Care Company in Gwinnett County, GA. May travel up to 200 miles per day. Mileage is reimbursed. 
 
Daily Responsibilities for Community Resource Coordinator - Field:
<ul>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Provides support to members to connect them to known community and care resources in a cost- effective manner
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Supports the coordination of community outreach resources available to members and promotes awareness of care/services
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Serves as support for members on community and care resource inquiries and opportunities available to members
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Supports all member related correspondence and educational materials to assist in the facilitation of a successful community connection
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt 0.5in;">Documents and maintains all community resources to ensure standards of practice and policies are in accordance with health plan requirements
</ul>
 
Required Qualifications for Community Resource Coordinator - Field:

<ul>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Located in Gwinnett County 
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Working Knowledge of Social Determinants of Health (SDOH) barriers 
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Working Knowledge of geographic area(i.e. posted county/service area) and known Community Based Organizations
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Working knowledge of motivational interviewing and care plan development/goal setting
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">General understanding of Microsoft office suite (i.e. work, outlook, excel)
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">HS Diploma or GED
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt 0.5in;">Clear and active Driver’s license, reliable transportation, and car insurance
</ul>
 
Preferred Qualifications for Community Resource Coordinator - Field:

<ul>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Bilingual highly preferred
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt 0.5in;">Community Health Worker Certification highly preferred 
</ul>
