Home Health Physical Therapist - Aurora, CO 
 
Looking to make an impact for patients and families in your community? HealthCare Support is seeking a Home Health Physical Therapist in Aurora, CO with a 4.5 STAR-rated healthcare provider. As a Home Health Physical Therapist, you’ll have the opportunity to provide personalized, one-on-one care to patients in their homes. 
  
Daily Responsibilities:
<ul>
	<li style="margin: 0.0in 0.0in 8.0pt;">Provide skilled in-home physical therapy care, evaluating patients’ mobility, safety, strength, balance, and functional abilities, then creating personalized treatment plans that help them recover independence after illness, injury, or surgery.
	<li style="margin: 0.0in 0.0in 8.0pt;">Work independently in the field while collaborating with the care team, delivering one-on-one therapy in patients’ homes, educating patients and families on exercises and safety techniques, and monitoring progress to ensure the best possible outcomes.
	<li style="margin: 0.0in 0.0in 8.0pt;">Typically seeing 3-6 patients/day depending on the type of visit.
</ul>
Required Qualifications:

<ul>
	<li style="margin: 0.0in 0.0in 8.0pt;">Minimum nine months’ experience as a physical therapist
	<li style="margin: 0.0in 0.0in 8.0pt;">Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
	<li style="margin: 0.0in 0.0in 8.0pt;">Currently licensed to practice as a physical therapist in Colorado
	<li style="margin: 0.0in 0.0in 8.0pt;">Must be licensed driver who can travel to all business locations 
</ul>
