RN Field Care Manager 
 
HealthCare Support is actively seeking a RN Field Care Manager to fill an opening in Detroit, Michigan.  
 
Daily Responsibilities for RN Field Care Manager:
<ul>
	<li>Must reside in Michigan with the ability to drive to Wayne or Macomb Counties.
	<li>This position will be based from a home office and will travel 75% of the time, to an assigned area in Wayne or Macomb county, to conduct in home visits with Medicare/Medicaid members.
	<li>The Field Care Manager Nurse 2 employs a variety of strategies, approaches, and techniques to manage a member&apos;s physical ,environmental, and psycho-social health issues.
	<li>Monday to Friday 8AM - 5PM
</ul>
Required Qualifications for RN Field Care Manager:

<ul>
	<li>Active Michigan Registered Nurse (RN) license with no disciplinary action
	<li>Must reside in the state of Michigan
	<li>Ability to travel to homes and community settings for face-to-face assessments
	<li>Experience working with the adult population, disease management
</ul>

Pay Details: $35- $40/ Hour
