Nurse Practitioner 
 
HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with Managed Care partnered SNF in Fairview Manor, PA. 
 
Daily Responsibilities for Nurse Practitioner:
<ul>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">Conduct patient facing comprehensive assessments between 4 SNF facilities to prevent unnecessary hospital readmissions for acute and chronic conditions</li>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">Conduct advanced illness and advanced care planning conversations for 4-5 patients/day</li>
	<li style="margin:0in 0in 8pt">Mon – Fri 8am-5pm</li>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">3-6 month conversion to FTE</li>
</ul>
 
Required Qualifications for Nurse Practitioner:

<ul>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">Graduate of an accredited Master of Science Nursing program</li>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">NY Board Certified Family, Adult, Gerontology or Adult-gerontology Acute Care NP</li>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">DEA Prescriptive Authority</li>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">Covid Vaccination</li>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">1+ years of clinical experience in NP Roles</li>
</ul>
 
Preferred Qualifications for Nurse Practitioner:

<ul>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">Doctor of Nursing Practice </li>
	<li style="margin-top:0in; margin-right:0in; margin-bottom:0in; margin:0in 0in 8pt">Meeting the medical needs of patients with complex behavioral, social and/or functional needs</li>
	<li style="margin:0in 0in 8pt">Understanding of Advanced Illness and end of life discussions</li>
</ul>
