Home Health LPN - $10K Sign-On Bonus

Looking to make an impact for patients and families in your community? HealthCare Support is seeking a Licensed Practical Nurse (LPN) in Grants Pass, OR and surrounding areas with a 4.5-star rated healthcare provider. As a Home Health LPN, you will have the opportunity to work at the top of your license while providing one-on-one care to patients in their homes.

Daily Responsibilities for the Home Health LPN / Licensed Practical Nurse:

<ul>
	<li>Provide follow-up care to patients after discharge from hospital, rehabilitation, skilled nursing, or long-term care facilities
	<li>Complete daily paperwork to maintain accurate and complete medical records
</ul>

Shift:

<ul>
	<li>Monday-Friday 8am-5pm
</ul>

Pay Details:

<ul>
	<li>Up to $39.85/hour
	<li>$10K Sign-On Bonus
</ul>

Required Qualifications:

<ul>
	<li>Active Oregon Licensed Practical/Vocational Nurse
	<li>Valid CPR certification
	<li>At least 1 year of acute care experience (Home Health, Med-Surg, ED, SNF, etc.)
	<li>Valid driver’s license and insured vehicle
</ul>
