Advanced Practice Clinician- Nurse Practitioner/Physician Assistant 
 
HealthCare Support is actively seeking an Advanced Practice Clinician- Nurse Practitioner or Physician Assistant&nbsp;to fill an opening in Greensboro, NC. 
 
Daily Responsibilities for NP/PA:&nbsp;
<ul>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Travel to 4 facilities in Guilford County to perform comprehensive patient assessments, addressing both medical and behavioral conditions.</li>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Manage acute and chronic illnesses proficiently, ensuring accurate and thorough documentation, including ICD 10 conditions.</li>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Communicate and collaborate with the interdisciplinary care team, conducting advanced illness and care planning conversations, while providing counseling and education to patients and caregivers.</li>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">M-F 8am-5pm</li>
</ul>
 
Required Qualifications for NP/PA:

<ul>
	<li style="margin: 0.0in 0.0in 0.0in 0.5in;">Active and unrestricted license in&nbsp;North Carolina</li>
	<li style="margin: 0.0in 0.0in 0.0in 0.5in;">Graduate of an accredited MSN, DNP, or MPA program</li>
	<li style="margin: 0.0in 0.0in 0.0in 0.5in;">Board certified NP (through ANCC or AANP) or PA</li>
	<li style="margin: 0.0in 0.0in 0.0in 0.5in;">Current, active DEA licensure/prescriptive authority</li>
	<li style="margin: 0.0in 0.0in 8.0pt 0.5in;">Full COVID-19 vaccination</li>
</ul>
 
Preferred Qualifications for NP/PA:

<ul>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">1+ year of clinical experience in practice (long-term care setting preferred)</li>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs</li>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Understanding of Geriatrics and Chronic Illness</li>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Understanding of Advanced Illness and end of life discussions</li>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Proficient computer skills including the ability to document medical information with written and electronic medical records</li>
</ul>
