Field RN Service Coordinator 
 
HealthCare Support is actively seeking a Field RN Service Coordinator to fill an opening with a leading managed care organization in South East Houston, TX. 
 
Daily Responsibilities for Field RN Service Coordinator:
<ul>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Duties: Responsible for performing telephonic or face-to-face clinical assessments for the identification, evaluation, coordination, and management of member's needs, including physical health, behavioral health, social services and long-term services and supports; Identifies members for high-risk complications and coordinates care in conjunction with the member and the health care team; Manages members with chronic illnesses, co-morbidities, and/or disabilities, to insure cost effective and efficient utilization of health benefits; Obtains a thorough and accurate member history to develop an individual care plan
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Shift: Monday – Friday, 8am to 5pm 
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt 0.5in;">Location: Field visits to patient homes in South East Houston, TX area. Candidate must be a resident in the area.

Required Qualifications for Field RN Service Coordinator:

<ul>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Active Texas RN license required
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt 0.5in;">3-5 years of experience in a Managed Care environment required working with individuals with chronic illnesses or disabilities 
	 
</ul>
Preferred Qualifications for Field RN Service Coordinator:

<ul>
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt 0.5in;">Service Coordinator experience or Star Plus experience PREFERRED 
</ul>

Pay Details: $40.00 per hour - $48.00 per hour
