Long Term Care Manager – Hybrid 
 
HealthCare Support is actively seeking a Long Term Care Manager – Hybrid to fill an opening with a Managed Care Organization in Lehigh Acres, Florida. 
 
Daily Responsibilities for Long Term Care Manager – Hybrid:
<ul>
	<li>Responsible for members gaining access to needed services through coordination and integration of medical and long term care services for the purpose of orientation, care plan development, assessment, and care coordination.
	<li>Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development
	<li>Monitor delivery of services and follow-up with members, caregivers, or providers through in person visits and telephonic contact
	<li>Travel: Monthly and quarterly member contact and will include 80% travel - Will receive IRS mileage rate
	<ul>
		<li>They try to keep the travel radius within 15-30 miles of where the candidate lives
	</ul>
	
	<li>Shift: M-F 8AM-5PM EST
	<li>This is a HYBRID position 
	 
</ul>
Required Qualifications for Long Term Care Manager – Hybrid:

<ul>
	<li>Bachelor’s degree
	<li>2+ years of case management experience and completing field visits
	<li>Home Health, Long Term Care or discharge planning exp highly preferred
	<li>Must be comfortable going into members’ homes for assessments and able to handle caseloads of 50-100 members
	<li>Must have experience with EMR
	<li>Must have a Valid and Active Driver’s license
</ul>
 
Preferred Qualifications for Long Term Care Manager – Hybrid:

<ul>
	<li>Bilingual Spanish
	<li>Knowledge of healthcare and managed care preferred
	<li>Home health, long term care exp or Discharge planning exp
	<li>BSN
	<li>Bilingual in English/Spanish
	<li>Child Welfare/Department of Children and Families
	<li>Dependency Case Manager
	<li>Child Protective Investigator
	<li>DCF Navigator
	<li>Adoption Case Manager
	<li>Targeted Case Manager
	<li>Community Agency Worker
</ul>

Pay Details: $28.00 to $33.00 an hour
