Compensation Notes:  $28 to $31 an hour (can flex to $27 if needed for 43% markup) 
Location:  Looking for candidates in McAllen, Mission. Field based (3 days in field, 2 remote), will be doing assessments in members homes and sometimes adult daycare facilities.  They will be conducting face to face assessments in the Hidalgo SDA, consisting of 10 counties, visits will be primarily in Hidalgo county, but could travel to Maverick, Webb, Zapata, Starr, Willacy, Cameron, Jim Hogg, Duval or McMullen counties 
Shift: M-F 8-5, overtime sometimes required 
Start Date: ASAP

Minimum Education/Licensures/Qualifications:

	<li>Speak/Read/Write English/Spanish highly preferred
	<li>Clear and active TX LVN license or Bachelor's degree
	<li>2+ years working with people over 21 with disabilities and vulnerable populations (will be working with members applying for long term support services such as attending adult day care or receiving personal assistant services to assist with activities of daily living)
	<li>Field/Home Health Experience,  experience completing assessments in the field required
	<li>Know and understand Medical Terminology and Disease Process
	<li>Experience working in a fast paced environment
	<li>HS Diploma or GED if they have LVN
	<li>Clear and active driver's license, reliable transportation and insurance
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 0.0in 0.5in;">Typing test and computer assessment required

Best Candidate: Bilingual LVN with 2+ years of care coordination in managed care 
Second Best Candidate:  Bilingual LVN with 2+ years in home health working with adults with chronic/complex conditions 
Third Best Candidate:  LVN with 2+ years of care coordination from MCO setting 
 
 
Day to Day Duties:

	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Work assigned tasks/assignments to address member needs/requests
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Work other possible assignments such as Complaints & Escalations
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Possible high priority assignments that will require urgent attention
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Update member record as needed
