SNF Case Manager / Licensed Vocational Nurse - Hybrid 
 
HealthCare Support is seeking a Licensed Vocational Nurse&nbsp;to join a mission-driven team in the&nbsp;Monterey Park, CA area.&nbsp;This full-time, salaried role offers autonomy in the field and remotely.

Schedule:

	<li style="margin:0in 0in 8pt">Monday – Friday, 8:30 AM – 5:00 PM - Hybrid</li>
</ul>
 
Compensation:

<ul>
	<li style="margin:0in 0in 8pt">$31/hr - $38/hr per hour&nbsp;(based on experience)</li>

Daily Responsibilities:

<ul>
	<li style="margin:0in 0in 8pt">Conduct daily outreach to facilities while assisting patient admissions and discharges.</li>
	<li style="margin:0in 0in 8pt">Assist with discharge planning and identifying post-discharge needs, while also coordinating with facility case managers to coordinate care transitions </li>
</ul>
 
Qualifications

<ul>
	<li style="margin:0in 0in 8pt">Active Licensed Vocational Nurse (LVN) license in California</li>
	<li style="margin:0in 0in 8pt">Experience using Microsoft applications such as Word, Excel and Outlook</li>
	<li style="margin:0in 0in 8pt">At least two (2) years of clinical experience as an LVN</li>
</ul>
