-Bilingual RN Case Manager 
 
HealthCare Support is actively seeking a Bilingual RN Case Manager to fill an opening in Orange, California.  
 
Daily Responsibilities for Bilingual RN Case Manager:

<ul>
	<li>Assesses member needs using a standardized health needs assessment or health risk assessment.
	<li>Reviews, modifies and updates care plans continuously to reflect the member’s needs, at minimum, annually or upon change in condition.
	<li>Coordinates care and services with members, members’ family members/representatives and other providers, as appropriate, including community supports and Long-Term Services and Supports (LTSS).
	<li>Monday to Friday 8AM - 5PM PST
</ul>
Required Qualifications for Bilingual RN Case Manager:

<ul>
	<li>Associates Degree in Nursing or related field REQUIRED
	<ul>
	</ul>
	
	<li>Bachelors preferred 
	<li>RN license in CA
	<li>Strong written and verbal communication skills 
</ul>
Preferred Qualifications for Bilingual RN Case Manager:

<ul>
	<li>MCO experience 
	<li>Case Management preferred
	<li>Certified Case Manager Certification (CCM)
</ul>
