Patient Care Manager / Registered Nurse&nbsp;

HealthCare Support is actively seeking an&nbsp;Patient Care Manager&nbsp;with one of the nation's leaders in post-acute home&nbsp;healthcare services&nbsp;in Ormond Beach, FL. 
&nbsp; 
Daily Responsibilities for the Patient Care Manager:

<ul>
	<li>Overseeing a team of&nbsp;clinicians and support staff</li>
	<li>Coordinate patient care, approve orders, resolve concerns, and ensure quality standards are met</li>
	<li>Lead team meetings, case conferences, and provide coaching, training, and performance support</li>
	<li>Monitor key performance indicators (KPIs), documentation, admissions, and regulatory compliance</li>
</ul>
Required Qualifications for the Patient Care Manager:

<ul>
	<li>Current&nbsp;Registered Nurse licensure&nbsp;in the state of practice</li>
	<li>1+ years of management or supervisory experience&nbsp;in a home care setting</li>
	<li>Licensed driver with a reliable, insured automobile per state and agency requirements</li>
</ul>

Pay Details:

	<li>$80-$90K (depending on experience)&nbsp;</li>
