Inpatient Case Manager / Licensed Vocational Nurse - Hybrid&nbsp; 
 
HealthCare Support is seeking a Inpatient Case Manager to join a mission-driven team in the&nbsp;Pasadena/Arcadia, CA area.&nbsp;This full-time, salaried role offers autonomy in the field and remotely. 
 
Territory:&nbsp;
<ul>
	<li>Pasadena, CA&nbsp;</li>
	<li>Arcadia, CA</li>
</ul>
Schedule:

<ul>
	<li>Monday – Friday, 8:00 AM – 5:00 PM</li>
	<li>Hybrid -&nbsp;2-3 days on site at hospital &amp; remaining days remote</li>
	<li>No nights, No on-call, and no weekends!&nbsp;</li>
</ul>
 
Compensation:

<ul>
	<li>$31-$38 per hour</li>

Daily Responsibilities:

<ul>
	<li>Ensure compliance with Case Management policies and health plan Utilization Management standards, including accurate medical necessity and level-of-care reviews using Milliman Care Guidelines.</li>
	<li>Coordinate inpatient case management activities by reviewing medical records, conducting initial and concurrent reviews, collaborating with providers and discharge planners, and supporting timely, appropriate discharge planning.</li>
	<li>Communicate determinations and case updates with medical directors, health plans, providers, and leadership while maintaining strong professional relationships and on-site hospital availability.</li>
</ul>
Qualifications

<ul>
	<li>Active Licensed Vocational Nurse (LVN) license in California</li>
	<li>Experience using Microsoft applications such as Word, Excel and Outlook</li>
	<li>At least two (2) years of clinical experience as an LVN&nbsp;</li>
</ul>
