Home Health LPN / Licensed Practical Nurse 
  
Healthcare Support is actively seeking a Home Health Licensed Practical Nurse to fill an opening for a top healthcare provider located in Scranton, PA. 
  
Daily Responsibilities for the Home Health LPN / Licensed Practical Nurse:

<ul>
	<li>Home healthcare nurses provide follow-up care to patients after they have been discharged from a hospital or rehabilitation, skilled nursing, or long-term care facility
	<li> Provide nursing visits in accordance with the written plan of treatment signed by the physician and all subsequent orders that authorize and direct changes
	<li>Provide (on a daily basis) paperwork as required by agency to maintain accurate and complete medical records
</ul>
Pay Details: 

<ul>
	<li>Position is Pay Per Visit however annual salary ranges between $55-62K for 32 hours per week
	<li>Shift: 32 hours a week – Monday – Friday | On call rotation is 1 week on every month Sun-Sat.
</ul>
 
Required Qualifications for the Home Health LPN / Licensed Practical Nurse:

<ul>
	<li>Currently Licensed Practical/Vocational Nurse in Pennsylvania
	<li>Must possess and maintain valid CPR certification while employed in a clinical role
	<li>At least 1-year acute care experience as an LVN (Home Health, Med-Surg, ED, SNF etc.)
	<li>Experience with IVs, Wound Care, and Disease Management (diabetes, CHF, liver disease, COPD)
	<li>Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order
</ul>
