RN Field Case Manager 
 
 
HealthCare Support is actively seeking a RN Field Case Manager  to fill an opening in Seattle, Washington. 
 
Daily Responsibilities for RN Field Case Manager
<ul>
	<li>Ensure accurate and complete documentation.
	<li>Communicate and collaborate with the interdisciplinary care team
	<li>Assist the provider/team with various care coordination activities
	<li>Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
	 
	<li>Monday through Friday 8AM - 5PM
</ul>
Required Qualifications for RN Field Case Manager

<ul>
	<li>Active and unrestricted RN license in Washington
	<li>Case management background preferred- experience working in a Long-Term Care facility as an RN preferred
	<li>Associate degree or higher in Nursing (RN)
	<li>Certified in Basic Life Support
</ul>
Preferred Qualifications for  RN Field Case Manager

<ul>
	<li>1+ year of clinical experience in practice (long-term care setting preferred)
	<li>Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
	<li>Understanding of Geriatrics and Chronic Illness
	<li>Proficient computer skills including the ability to document medical information with written and electronic medical records
	<li>Effective Communication skills and time management
