Job Description: 
 
The PACE Licensed Dietician (LD) is a mission-driven role focused on improving the health and quality of life for older adults participating in the PACE Program. The role of the LD is to function as a key member of the Interdisciplinary Team (IDT) and work with PACE participants to provide nutritional interventions and education. The role coordinates the dietary services of the Day Health Center and works with participants of high medical complexity in collaboration with primary care practitioners to optimize quality of life, while honoring what matters most. 
Essential Responsibilities:

	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Conduct comprehensive nutritional assessments of participants to identify their dietary needs and preferences across the continuum, including occasionally visiting home and community settings.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Actively participate in the IDT by offering clinical expertise in team planning, problem-solving, and processes.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Develop and implement individualized nutrition care plans to address participants’ health conditions and dietary goals.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Assess nutritional needs, diet restrictions, and current health plans to develop and implement dietary care plans and provide nutritional counseling.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Advise participants and their families on nutritional principles, dietary plans, diet modifications, and food selection and preparation in a manner which honors goal-concordant care.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Provide education and counseling to participants, families, and caregivers on healthy eating habits and nutrition management to improve their quality of life, including but not limited to: diabetes education, heart failure education, renal disease education, and blood glucose and fluid management.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Promote wellness and increase awareness and understanding of self-management.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Inspect meals served for conformance to prescribed diets and standards of palatability and appearance.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Coordinate recipe development, work with food vendors, and standardize and develop new menus for food service operations to meet the needs of participants.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Collaborate closely with other healthcare professionals, including physicians, nurse practitioners, physician associates, nurses, social workers, and therapists, to provide integrated care.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Accurately document in the Electronic Health Record (EHR) in a timely manner Provide all services in a manner consistent with State and Federal regulatory requirements.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Serve on various internal and external committees as required or designated.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Participate in team meetings, caregiver groups, and special events as needed.
	<li style="margin-bottom: 0.0in;margin: 0.0in 0.0in 8.0pt;">Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
