Nurse Practitioner / Physician Assistant&nbsp; 
 
HealthCare Support is actively seeking a&nbsp;Nurse Practitioner / Physician Assistant&nbsp;&nbsp;to fill an opening Windsor/ Enfield,&nbsp;Connecticut. 
 
Daily Responsibilities for Nurse Practitioner / Physician Assistant:
<ul>
	<li>Conduct comprehensive assessments</li>
	<li>Effectively manage acute and chronic medical and behavioral conditions</li>
	<li>Ensure accurate and complete documentation, including ICD 10 conditions</li>
	<li>Monday to Friday&nbsp;8-5pm</li>
</ul>
Required Qualifications for Nurse Practitioner / Physician Assistant:

<ul>
	<li>Active and unrestricted license in Connecticut as an NP or PA</li>
	<li>NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program</li>
	<li>NP/ PA&nbsp;Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center</li>
	<li>Current, active DEA licensure/prescriptive authority</li>
</ul>
Preferred Qualifications&nbsp;for Nurse Practitioner / Physician Assistant:

<ul>
	<li>1+ year of clinical experience in practice (long-term care setting preferred)</li>
	<li>Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs</li>
	<li>Understanding of Geriatrics and Chronic Illness</li>
</ul>
