Senior UAS Assessor – Onondaga County 
 
HealthCare Support is actively seeking a Senior UAS Assessor to support members in Onondaga County, NY. This organization is known for its commitment to high-quality, community-based care management and its focus on accurate, timely assessments to support long-term services and supports. In this role, the assessor will conduct comprehensive UAS-NY assessments, collaborate with care teams, and help ensure members receive appropriate, high-quality services—offering a meaningful position with direct impact on patient outcomes. 
 
Schedule:

<ul>
	<li>Monday – Friday, 8:30 AM – 5:00 PM&nbsp;</li>
</ul>
 
Compensation Notes:

<ul>
	<li>$90,000–$95,000/Annually&nbsp;</li>
</ul>

 
Responsibilities of a Senior UAS Assessor

<ul>
	<li>Assess and coordinate medical and support services across the continuum of care for complex, high-acuity populations to promote quality, cost-effective outcomes.</li>
	<li>Develop and manage personalized long-term care plans while educating members and families on available services and benefits.</li>
</ul>

Required Qualifications&nbsp;of a Senior UAS Assessor

	<li>Active and clear NY State RN license</li>
	<li>Experience in UAS, hospital, skilled nursing, or home health settings</li>
