Senior UAS Assessor - Albany, NY 
 
HealthCare Support is actively seeking a full-time Senior UAS Assessor to support members in Albany, NY. This organization is known for its commitment to high-quality, community-based care management and its focus on accurate, timely assessments to support long-term services and supports. In this role, the assessor will conduct comprehensive UAS-NY assessments, collaborate with care teams, and help ensure members receive appropriate, high-quality services—offering a meaningful position with direct impact on patient outcomes. 
 
Schedule:
<ul>
	<li>Monday – Friday, 8:30 AM – 5:00 PM
	<li>Work-life balance: No weekends, no holidays, no night shifts
</ul>
 
Compensation Notes:

<ul>
	<li>$90,000 – $97,000 / Annually
	<li>Annual Bonus Incentive 
	<li>Mileage & toll reimbursement
</ul>
 
Daily Responsibilities for Senior UAS Assessor:

<ul style="margin-bottom: 0.0in;margin-top: 0.0in;unicode-bidi: embed;">
	<li>Assess and coordinate medical and support services across the continuum of care for complex, high-acuity populations to promote quality, cost-effective outcomes.
	<li>Develop and manage personalized long-term care plans while educating members and families on available services and benefits.
</ul>
 
Required Qualifications for Senior UAS Assessor:

<ul style="margin-bottom: 0.0in;margin-top: 0.0in;unicode-bidi: embed;">
	<li style="margin-top: 0;margin-bottom: 0;vertical-align: middle;">Active and clear NY State RN license
	<li style="margin-top: 0;margin-bottom: 0;vertical-align: middle;">4+ years of recent experience as a RN 
	<li style="margin-top: 0;margin-bottom: 0;vertical-align: middle;">Must have Home Health or UAS experience
