Patient Assessment - Conduct comprehensive evaluations of patients’ medical, psychological and social needs, identify barriers to care and determine necessary services
Care Planning - Develop individualized care plans tailored to each patient’s condition and goals, collaborate with physicians, therapists, social workers and other healthcare providers
Care Coordination - Schedule appointments, treatments and follow-up services, ensure smooth transitions between care settings (e.g., hospital to home or rehab)
Patient Advocacy - Serve as a liaison between patients, families, providers and insurance companies, advocate for patient needs, preferences and rights throughout the care process
Education and Support - Educate patients and families about diagnoses, treatment options and self-care strategies, provide emotional support and guidance during recovery
Monitoring and Evaluation - Track patient progress and adjust care plans as needed, document all interactions, assessments and outcomes accurately
How to Work as an RN Case Manager
An RN Case Manager must be able to multitask and handle numerous responsibilities throughout each day. They must also be effective communicators with patients, families, doctors and insurance providers; capable of understanding and explaining the features of Medicare, Medicaid and other insurance options.
RN Case Managers are required to have an associate or bachelor degree from an accredited nursing school. Topics studied generally include anatomy, psychology, physiology, behavioral science, nursing responsibilities and more.
Skills and qualifications needed:
Active RN license
Clinical experience (typically 2–5 years)
Strong communication, critical thinking and organizational skills
Certification such as Certified Case Manager (CCM) is often preferred
